About Summit ElderCare :

Fallon Health operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Called Summit ElderCare, Fallon’s PACE helps provide older adults and their caregivers with a welcome alternative to nursing home care. Participants in Summit ElderCare have access to comprehensive medical services and social support at a Summit ElderCare site while they keep the independence of living in their own homes and communities.

Brief Summary of Purpose:

The Behavioral Health Clinician provides Behavioral Health services in support of SE participants’ health and well-being, and to promote effective caregiving and self-care skills in participants’ caregivers and SE staff. Looking for candidates

with trauma-informed care and substance use disorder treatment, and bilingual/Spanish- speaking preferred.

Responsibilities

  • Provides behavioral health support to Summit participants as referred by the IDT:

  • Conducts psychosocial, behavioral screenings and assessments of program participants and writes professional reports in the EMR

  • Participates in the development of an interdisciplinary treatment plan appropriate to participant’s goals and level of functioning, mental health or cognitive status, and care needs.

  • Makes recommendations for appropriate treatment to community MH specialists.

  • Reviews updates from Community-based Mental Health providers to support the most effective Mental Health treatment and services for individual PACE participants.

  • Provides individual counseling/coaching to PACE participants (caseload of 15-17 direct client contact hours per week) with the opportunity to facilitate psychoeducational and therapeutic groups

  • Collaborates with the Interdisciplinary Team to develop and implement appropriate behavior change interventions. Reviews requests and re-authorizations for community-based mental health provider services:

  • Participates in IDT meetings

  • Facilitates monthly BH Rounds with the PACE team to review participants on active BH services

  • Meets with team members to discuss BH cases as needed

  • Attends acute-care or rehab discharge planning meetings as needed

  • Attends family meetings as appropriate

  • Attends care planning meetings as needed and makes BH recommendations for inclusion in the PACE care plan

  • Facilitates case referrals for team consultation to Summit psychiatry consult program

Qualifications

Masters Degree in Counseling Psychology, Clinical Psychology, Applied Psychology, Rehab Psychology, Mental Health, or Social Work; or Doctorate in Counseling or Clinical Psychology

Massachusetts Licensure for Independent Practice: LMHC, LMFT, LICSW, Licensed Psychologist

  • Experience in community-based behavioral health practice

  • At least one year of experience working with older adults, preferred.

  • Experience providing and willingness to learnevidence-based practice models for treatment of mental health disorder, based in trauma-informed care

  • Understanding of the physiological, psychological and neuropsychological contributors to behavior in diverse clinical population.

  • Ability to use clinical judgment to provide appropriate and effective treatment

  • Maintains professional and ethical counseling standards, including client confidentiality and HIPAA standards

  • Able to work as a team member.

  • Access to reliable transportation to carry out community assignments is required

  • Excellent written and verbal communication skills

  • Excellent organization and time management skills and ability to complete record-keeping in a timely manner required.

  • Facile with computer applications including an Electronic Medical Record and Microsoft products.
