About Summit ElderCare :
Fallon Health operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Called Summit ElderCare, Fallon’s PACE helps provide older adults and their caregivers with a welcome alternative to nursing home care. Participants in Summit ElderCare have access to comprehensive medical services and social support at a Summit ElderCare site while they keep the independence of living in their own homes and communities.
Brief Summary of Purpose:
The SE Intake LPN will be responsible for conducting assessments of seniors in their homes throughout the service area to evaluate their medical needs, cognitive and physical functioning and limitations as well as their safety in their current living environment. In addition, the Intake LPN will work with the Intake SW and the intake team to determine what level of PACE services may be needed if the individual were to enroll in PACE. Works collaboratively with and is an extension of the PACE clinical team.
Primary Job Responsibilities:
Works closely with Enrollment Coordinator and Intake SW to schedule home visits with prospective PACE enrollees and their families/caregivers.
Along with the Intake SW and Enrollment Coordinator RN, during the home visit, assesses the prospect’s eligibility for PACE and their safety in the current living environment (i.e., assesses their ability to perform certain activities of daily living like bathing, dressing, toileting, meal prep, etc.).
Reviews with the prospect/their caregiver all currently prescribed and OTC medications as well as current medical problems
Reviews with the prospect/their caregiver all current providers including PCP and any specialists currently following the prospect and for what conditions
Reviews with prospect/their caregiver any history of falls, surgeries, ER visits and hospitalizations
Meets with Intake SW, Site Director and Provider to review findings and recommendations and participates in making enrollment decisions for PACE prospects.
In the instance of an acceptance for enrollment, collaborates with the Intake team (Intake SW, Site Director and Provider) to determine an initial PACE service package for the prospect and documents this in Nextgen
Documents all findings and enrollment decision outcomes thoroughly in Nextgen
Coordinates and schedules, in collaboration with Intake SW, Service Planning meetings with approved prospects
Education, Licenses, Certification, and Experience Requirements:
Graduate of an Accredited Practical Nursing Program
Valid license to perform Practical Nursing in the Commonwealth of Massachusetts
Access to reliable transportation to carry out essential functions of this role
Certification in CPR/Healthcare Provider issued by The American Heart Association or other accredited agency required.
Three years’ nursing experience, preferably in home care, long term care or community setting
At minimum, one year nursing experience working with frail elders
